STUDENT CONSENT FORM DRIVING LESSONS
1. PARENTAL CONSENT (IF UNDER 18)
I, the undersigned parent/guardian, give full consent for my child to participate in driving lessons with U-Drive It Driving School. I understand this includes both classroom instruction (if applicable) and behind-the-wheel driving sessions.
2. REQUIRED PAPERWORK
I acknowledge that all required documentation (proof of learner's permit/license, identification, and any other applicable forms) must be submitted prior to the start of lessons. Lessons will not proceed without proper paperwork.
3. VEHICLES
Driving lessons will be conducted in vehicles owned, maintained, and insured by U-Drive It Driving School. Students are not permitted to use personal vehicles for instruction unless specifically authorized.
4. STUDENT'S PHYSICAL & MENTAL CONDITION
The student must be in good physical and mental health during each lesson. The parent/guardian affirms that the student is free from any medical, physical, or mental condition that could impair safe driving.
5. BEHAVIOR & CONDUCT
Students must follow all instructor directions and behave respectfully at all times. Disruptive, unsafe, or disrespectful behavior may result in immediate termination of the lesson without refund.
6. LATE CANCELLATION
Any driving lesson canceled or rescheduled with less than 4 hours’ notice will incur a $35 late cancellation fee. This fee must be paid in full before the student may schedule or attend any further lessons. Failure to pay the late cancellation fee may result in suspension of future driving lessons until the balance is cleared.
7. COGNITIVE AWARENESS
Students must be alert, attentive, and free from the influence of alcohol, drugs, or any impairing substances during lessons.
8. EYESIGHT
Students must meet Florida's minimum vision standards for driving and must wear corrective lenses if required.
9. WARRANTY
U-Drive It Driving School does not guarantee that a student will pass the road test or become a safe driver. Instruction is intended to improve skills but ultimate driving ability depends on the student's commitment and practice.
10. REFUNDS
Refunds will not be issued for missed lessons without a minimum of 24 hours' notice. No-shows or late arrivals will forfeit the lesson fee.
11. RELEASE & WAIVER
I release U-Drive It Driving School, its owners, employees, and instructors from any liability for injuries, accidents, or damages occurring during lessons, except where caused by gross negligence.
12. PERSONAL PROPERTY
The school is not responsible for lost, stolen, or damaged personal belongings. Students may not use mobile phones, headphones, or other electronic devices during lessons.
13. PARENT/GUARDIAN SHADOWING
For safety and instructional purposes, parents/guardians are not permitted to ride in the vehicle during driving lessons.
14. MEDIA
The school may take photos or videos for training or promotional purposes. Please indicate consent:

15. DANGERS & DAMAGES
Driving inherently carries risks. I understand and accept that accidents, injuries, or vehicle damage may occur. I agree to be responsible for any intentional damage caused by the student.
16. FEES
All lesson fees must be paid in full prior to the scheduled lesson unless otherwise arranged.
17. CLOTHING
Students must wear appropriate clothing and closed-toe shoes. Lessons may be canceled without refund if the dress code is not followed.
18. WEATHER CANCELLATIONS
In Florida, heavy rain may be prevalent and the instructor has the right to cancel the driving lesson if conditions warrant.
